The Gut: Our Forgotten Organ of Wisdom
In 1988, on an ordinary summer day in Sydney, Australia, a middle-aged woman lay in a hospital bed at St. George Hospital, less than ten kilometers from the city center. Her eyes reflected an ineffable emotion—the complex intertwining of despair and hope. She had battled depression for nearly two decades, having tried virtually every available treatment: from selective serotonin reuptake inhibitors (SSRIs) to cognitive behavioral therapy, from electroconvulsive therapy to intensive inpatient care. Her condition resembled a stubborn undercurrent, always resurging after brief improvements, dragging her repeatedly into the abyss. Yet this very patient, who had lost confidence in life, would become a witness and participant in a pivotal moment in medical history—she was about to undergo a treatment that seemed almost absurd at the time: Fecal Microbiota Transplantation (FMT).
The gastroenterologist who administered this treatment was Thomas Borody, an Australian digestive system specialist practicing in Sydney. Borody was not an unknown figure in the academic circles of his time, but his research direction—the relationship between gut microorganisms and intractable diseases—remained in the marginal domains of mainstream medicine. Most researchers of that era were more inclined to focus on the role of gut microorganisms in infectious diarrhea, dismissing the hypothesis that they might influence brain function and mood as pseudoscience. However, it was precisely this "heterodox" research direction that would eventually撬动 a multi-billion dollar industry, spawn countless clinical trials, and fundamentally transform humanity's understanding of the "self."
When Borody slowly infused a suspension of feces from a healthy donor through a nasogastric tube into this female patient's digestive tract, he perhaps did not foresee that this attempt, derided by colleagues as "fecal therapy," would become a strong candidate for the New England Journal of Medicine's breakthrough therapy of the year thirty-five years later. More significantly, this patient's depressive symptoms showed remarkable improvement in the following weeks—not partial remission, but a near-dramatic reversal. She regained interest in life, could communicate normally with her family, and even began planning future work and travel. This case, like a stone dropped into stagnant water, rippled through the medical community, eventually converging into a scientific revolution regarding the gut-brain axis.
Thomas Borody and the Origins of Microbiota Transplantation
To understand why Borody embarked on this seemingly uncertain research path in the late 1980s, we need to rewind slightly to earlier medical practice scenarios. Borody received medical training in London and, after moving to Australia in the 1980s, dedicated himself to the field of digestive system diseases. Among the conditions in his clinical practice, one category drew his particular attention: recurrent Clostridioides difficile infection (CDI). Clostridioides difficile is a bacterium capable of causing severe diarrhea and colitis. When patients use antibiotics for other infections, this drug-resistant bacterium often seizes the opportunity to proliferate massively, leading to recurrent infections. Traditional treatments—antibiotics targeting C. difficile such as vancomycin or metronidazole—can control acute symptoms but have extremely high recurrence rates. Some patients experience dozens of recurrent infections, suffering endlessly.
During his treatment of these patients, Borody noticed a phenomenon: patients with concurrent constipation or other intestinal motility dysfunction had significantly higher recurrence rates of C. difficile infection. He began contemplating a seemingly simple yet profound question: if the imbalance of the gut microbiota led to the overgrowth of C. difficile, could restoring the balance of the microbiota fundamentally solve this problem? This thinking led him toward a bold hypothesis—using healthy donors' feces to rebuild the patient's gut microbiota.
1988 marked a turning point for Borody. After successfully treating several cases of recurrent C. difficile infection, he began expanding the application of this technique to other diseases associated with gut microbiota dysbiosis. He first attempted using FMT to treat inflammatory bowel disease (IBD), including ulcerative colitis (UC) and Crohn's disease (CD). IBD is a chronic intestinal inflammatory disease whose pathogenesis involves the complex interplay of genetic factors, immune factors, and environmental factors. In that era, mainstream treatment focused on suppressing immune responses, such as using glucocorticoids, aminosalicylates, or immunosuppressants, but these treatments often came with significant side effects and could not achieve cure.
Borody's approach differed entirely from mainstream views. He believed that since the gut microbiota of IBD patients differed significantly from that of healthy individuals, and these differences might be not only the result but also the cause of the disease, introducing healthy donors' microbiota through FMT might correct this dysbiosis and improve inflammatory conditions. Between 1988 and 2000, Borody and his team conducted extensive FMT clinical practice in Australia, accumulating rich case experience. Although most of these early works were published in relatively obscure journals as case reports or small-scale cohort studies, they laid an important clinical foundation for later Randomized Controlled Trials (RCTs).
