Let's start with the core secret—understanding the "key and lock" relationship between drugs and diseases!
Drugs: The Body's Guardian Keys
Inside our bodies, there is an extremely complex yet orderly "little city"—our cells. In this city, countless "workers" are on duty 24/7, and they are mainly Proteins. These protein workers are responsible for transporting nutrients, sending signals, breaking down waste, building tissues, and more. When disease strikes, it's often because one or several "workers" have malfunctioned—they might be working too crazily (like certain growth signal proteins in some cancers), or simply go on strike (like missing key enzymes in some genetic diseases), or are controlled by "bad guys" (like viruses or bacteria). These malfunctioning proteins directly related to the disease are the "locks" our drugs need to find, professionally called Targets.
So, what is a drug? A drug is that carefully designed "key." This key must be able to precisely fit into the "lock's" keyhole (professionally called the Binding Site) and turn it in the right way—either stopping a "bad worker" from doing harm (like blocking a protein a virus needs to replicate), or fixing a "good worker's" function (like supplementing a missing enzyme), or removing the "bad worker" (like marking cancer cells for the immune system to clear). The "tooth shape" (chemical structure) of this key must perfectly match the "lock's" shape. That's why the first and most crucial step in drug discovery is finding the right lock (Target Validation). If you find the wrong lock, even the most exquisite key is useless and might even fit into other locks, causing unnecessary trouble—that's the origin of Side Effects. So, drug discovery is all about the ultimate pursuit of "perfect matching."
Let's understand with an example. Suppose there's a "bad lock" that makes cells grow out of control, called an "Abnormal Growth Factor Receptor." If a designed drug molecule (key) can fit precisely into a specific spot on this receptor, it can tell it: "Stop sending growth signals!" In this way, out-of-control cancer cells might be controlled. But if our key's tooth shape is wrong, it might not only fail to stop the abnormal receptor but also accidentally activate another important heart protein receptor, causing heart problems—that's a serious side effect. Therefore, scientists spend a lot of time on "finding the right lock" and "designing the right key."
The Six Major Stages of Drug Discovery
After finding the right "lock" (target), the long and arduous journey of "making the key" officially begins. Modern drug discovery usually follows a process that, while not perfectly linear, is logically clear. We can think of it as building a bridge to the shore of health, requiring six key "piers" or stages. The flowchart below shows the complete journey from disease to drug. Take a close look, and we will unpack the story of each pier below.

*This is the starting point of all journeys, like a detective receiving a case and first thoroughly understanding the "culprit" (the disease). Scientists—including doctors, biologists, geneticists—will deeply study every aspect of a disease: What causes it (virus, bacteria, gene mutation, lifestyle)? What happens at the cellular and molecular level? Which protein "workers" have the most critical abnormal behavior? They look for proteins highly correlated with the disease state by observing patient samples, performing genomic analysis, and simulating the disease in the lab using cells or simple model organisms (like fruit flies, zebrafish). This process can last years, requiring extensive observation, hypothesis, and verification. Key Output: A list of one or more suspected "bad lock" target candidates.*
*Having found a suspect (candidate target), the next step is to ensure the evidence is solid—that it is truly the "mastermind" behind the disease. If target validation fails, all subsequent work will be in vain. Scientists use various "high-tech detective tools" for validation: they can directly increase or decrease the amount of this target protein in cells and see if disease-related behaviors change accordingly (e.g., do cancer cells stop proliferating after reducing a certain protein?); they can also use gene editing technologies (like CRISPR) to knock out or modify this target gene in animal models (like mice) and observe if the animals become disease-free or show reduced symptoms.
