Chapter 1: The Invisible Battlefield: The Million-Strong Army in Your Body
> > 【English】On a rainy Monday morning in spring 2024, Lin Xia, a 32-year-old internet operations specialist who had pulled three consecutive all-nighters to finish a project report, was dragged awake by a scorching pain in her throat the moment she lay down. She frowned and swallowed, wincing sharply at the jolt of pain: it felt like someone had smeared crushed glass shards on her tonsils, and even breathing carried a burning sensation. She got up in a daze to look for cold medicine, complaining about her recent bad luck, completely unaware that at this very moment, a microscopic war with a scale exceeding one trillion units had already entered its second round deep in her throat.

> > 【English】Have you ever had similar experiences? You accidentally cut your finger while cutting fruit, and the moment a drop of blood oozed out, you panicked a little, but the bleeding stopped in less than ten minutes. A few days later, the wound scabbed over, the scab fell off, and only an almost invisible shallow scar was left, and it itched so much during healing that you didn’t know whether to scratch it or not. Or maybe your arm was sore for two or three days after getting the COVID-19 vaccine, and then you found that everyone around you got infected, but you were fine. Or you sneezed when the seasons changed, had a runny nose for two days, and then recovered on your own without taking any medicine. These “small things” you take for granted are actually traces of the army in your body fighting.

> 【English】Comparing the human body to a city is not just literary rhetoric, but the most classic analogical model in immunology: the “residents” of this city are 37 trillion human cells, while the “army” guarding it is 1-2 trillion immune cells, equivalent to one soldier for every 15 residents, patrolling and guarding 24 hours a day. Its “city walls” are the skin covering the whole body, the “moat” is the mucosa in the respiratory tract, digestive tract, and urogenital tract, the “sentries” are immune organs distributed all over the body, the “soldiers” are various immune cells, the “weapons” are antibodies, cytokines, complements and other immune active substances, and the “military command” is the hypothalamus in the brain and the whole body’s immune signal network. > > The precision of this defense system far exceeds any military fortification built by humans so far: your epidermis is stacked with 10-20 layers of dead keratinocytes, filled with lipids composed of ceramides, cholesterol and free fatty acids between cells, equivalent to the structure of “bricks + high-strength cement”, which can block the invasion of almost all pathogens; the sebaceous glands on the skin surface secrete weakly acidic sebum, with a pH value maintained between 4.5 and 6.5, which can inhibit the growth of more than 90% of bacteria and fungi; even the thinnest eyelid mucosa is covered with cilia that beat 1000 times per second, which can sweep out dust and pathogens stuck on it like sweeping the floor. > > What’s more exaggerated is your digestive tract: this is the largest channel for pathogen invasion. The food you eat every day may be mixed with 100,000 to 1 million bacteria, viruses and parasite eggs, but 99% of them will die in gastric acid – the pH value of gastric acid is as low as 1.5-2, which is as corrosive as the electrolyte in car batteries, and can instantly dissolve the cell walls of most microorganisms. Even if a few invaders sneak into the intestine, they will be neutralized by secretory immunoglobulin A (sIgA, the main antibody on the mucosal surface) secreted by the mucosal surface, or have no living space because they are squeezed out by the symbiotic flora in the intestine.

> 【English】#### 1. Throat burning: immune cells are charging > Going back to Lin Xia’s example: the culprit that made her so painful that she could hardly swallow was not the rhinovirus itself – rhinovirus is a tiny virus that specifically infects the epithelial cells of the upper respiratory mucosa, and it only causes mild necrosis of the epithelial cells, which will not produce severe pain at all. What really made her gasp in pain was the “defensive reaction” of the immune system: when the rhinovirus breaks through the mucosal barrier and infects the epithelial cells, the damaged epithelial cells will release a series of signaling molecules, namely cytokines, such as interleukin-1 (IL-1), interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α). These cytokines do three things: first, they act on the thermoregulatory center of the hypothalamus to raise the body temperature, that is, fever – high temperature can inhibit the reproduction speed of most viruses; second, they dilate the blood vessels in the infected area and increase their permeability, so that the blood flow w
